The South Carolina Department on Aging, along with partnering organizations, is collecting input on the ways in which people learn about and use long term services and supports. Long term services and supports are those types of services that help people with health conditions or disabilities maintain their independence and remain in the home of their choice for as long as possible. For example: ramps, personal care with bathing or dressing, transportation assistance, home delivered meals, etc.

Please complete this survey if you are:

* An adult who uses, or who may benefit at some point from long-term services and supports.

* A friend or family member of someone with a disability or a health condition; or

* A paid provider of any type of long-term services or supports.

Your anonymous input will be used by the South Carolina Department on Aging to inform improvements to South Carolina’s long-term services and supports’ system.
